Meet Kent Hoffman, D.O., P.A.
Dr. Kent Hoffman is the medical director of Lake Howell Health Center. He is board-certified in family practice and addiction medicine. As a doctor of osteopathy, he studies your medical history and takes a whole-body approach to treatment. Dr. Hoffman also created the SoberDoc treatment program, which allows people struggling with addiction to recover successfully without losing their family, home, or job.

Nicole Floyd Palm is a Physician Assistant at Lake Howell Health Center, certified by the NCCPA. She has been practicing internal medicine for the past 8 years and addiction medicine for 4. She works with Dr. Hoffman to provide excellent care and a whole-patient approach to health and wellness.

Michelle Garcia, APRN, is a compassionate and board-certified APRN with deep roots in Central Florida. With a Master of Science in Nursing and a certification from the American Red Cross, she brings diverse clinical experience and a commitment to patient-centered care. Known for her dedication and trusted by both patients and peers, Michelle provides individualized, high-quality care to individuals and families throughout her community.
